To complete your registration, please follow the directions below.
To register students:
1. Look over the following items:

Check the On-Line Policy Acceptance--BOTH STUDENT AND GUARDIAN
--Log into the myDSD system under the guardian account
--Tools
--Sign documents
--Select files in the file selection drop down list
--Click accept for each document

Student Demographic Form -- IF changes are needed, print this form from within your my.DSD login.
To print form, click the myDSD Link and follow the directions below:
~Login with your User Id and password (Must be a guardian login--NOT a student id and pin)
~Click on the student you are registering (This is found in the available box on the left side of the screen)
~Click on Reports
~Click on Student Demographic form
~Print this form. There should be two pages
~Please update Demographic Form and return to Main Office
2. Complete any optional forms you may need:
Free and Reduced Lunch Information: Free and Reduced Lunch Form is a separate form from the Fee Waiver and both must be submitted separately.
Fee Waiver---Must be completed by October 4, 2020. All fee waiver applications are due no later than October 4, 2020. Fee Waiver applications must be submitted on a yearly basis.
Parking Permit--Parking passes will be sold in the main office starting August 13 (All U's and past fees must be cleared before obtaining a pass.) Because of limited space, Juniors and Seniors ONLY.
Medication Forms--Sign and bring it in form if you want to store medications of give permission to use medication at school.
Media--Sign and bring in the form if you do not want your student to be in any sort of media from the school.
Housing Information Form--Fill out this form if you think your student may be eligible for services under the McKinney-Vento Act (Homeless Assistance). English Spanish
3. Remember to visit the PSTA site and sign up for membership, volunteering and information about the upcoming school year.

5. Registration fees must be paid no later than September 27, 2019. For questions, please call 801-402-3900. A $10 late fee will be assessed to any student paying fees after September 27th. All Class Fees will be attached to students on August 30th.

7. Schedules will be available online through myDSD the week of August 7th.
Class Change Dates and Times:
-Students can view their schedules online on August 7th
-Counselors will NOT be responding to EMAILS or PHONE calls regarding class changes. Students/Parents MUST SIGN UP FOR AN APPOINTMENT to make class changes.
-All class changes will be $10. If the counselor deems there is no charge, your money will be refunded to you after your appointment.
-CLASS CHANGES BY APPOINTMENT: Please come on time and prepared for your appointment. Appointments will be 5 minutes each. If you are late, you will miss your appointment.
--SENIORS: August 12th - August 13th -- 8:00 am - 1:00 pm
--ANY STUDENT: August 14th, 15th and 16th -- 8:00 am - 1:00 pm
August 19th -- 8:00 am - 11:00 pm

Other Important Class Schedule Information: Please Read
--If you would like to take a class offered at another high school (magnet class), email your counselor in August. You will not be allowed to take classes at another high school if Bountiful High offers the course.
--To sign up for a home release period, you will need to set an appointment on myDSD with your counselor and bring a parent to the meeting to sign the appropriate form.
8. Pictures will be taken at the school on August 26th and 27th during school hours. Make-up pictures will be taken on October 7th.

Pictures will be taken at the start of the school year
Pictures will be taken for every student for their activity card and for their picture in the year book. Picture packets are optional. Pictures must be paid for on picture day in order for you to receive a packet.
Seniors you have received a mailing from Life Touch (Prestige) giving you a time for a photo session. At this photo session, they will take your yearbook picture as well as giving you the opportunity to have pictures taken in any other outfits that you would like to have pictures taken in. THIS IS OPTIONAL. You can have your picture taken for yearbook on the above dates at the high school.

Parking
PARKING PERMIT VEHICLE REGISTRATION
Students desiring to park a car in Bountiful High parking lots must purchase a parking permit. Semester parking permits will be sold to students for $5.00.
Permit must be visible at all times while parking on the BHS lot.
Due to limited parking, BHS Parking Passes will only be available for purchase to the following:
Juniors and Seniors--NO Sophomores
Students with NO U's
Students with NO fees from previous years owing
Parking Permits can be purchased in the Main Business Office beginning August 15th. Students can fill form out at the school.
Park only in designated spaces allowed by the permit. DO NOT PARK in Faculty, Visitor, Handicap, Day Care, Seminary, Bus Zones and Fire Lanes. Students whose vehicles are parked crooked across the stalls will be ticketed. Bountiful High School is not responsible for damage or loss to student property. Please lock your vehicle at all times.
Fee Waivers

New Students
Welcome to Bountiful High School. As a new student, you will need to complete the registration packet and then schedule an appointment to register. New student registration will be held on August 9th and 10th from 8:00 to 11:00 and August 22nd from 1:00 to 2:30. Students with the last name beginning R-Z, appointment times will be August 9th and 10th from 1:30 to 3:00, and August 22nd from 12:00 to 1:30. You can call Mrs. Bouchard in the counseling office at 801.402.3907 beginning August 7th to schedule your appointment. Office hours are 7 am to 3 pm
Registration packets can also be picked up in the Main Office at the school. During the summer new student paperwork can also be turned into the Main Office and an appointment can be made at that time.
You will need proof of residency, a birth certificate, guardian photo ID, immunization record and a transcript. Please click on the new student enrollment packet to see a listing of all the acceptable forms you will need to bring.

Students living with someone other than their parent: A petition for guardianship must be completed and approved through the District Office. Call Student Services 801.402.5159 for an appointment.
